Please review these findings and fix the issues before merging.
Also drop the leading slash from the vault secret path: v4 parses secret paths more strictly (leading slashes are reserved for engines like cubbyhole), so /providers/... becomes providers/... to keep the approle token fetch resolving correctly under v4. vault-action v4 + leading-slash fix; fresh CI green (stale June scans cleared). Vault validated post-merge.
