Allow mkdocs-pandoc to support structured pages. For example, the following pages structure
```yaml
pages:
- Section 1: section-1.md
- Section 2:
- Section 2.1: section-2/section-2-1.md
- Section 2.2: section-2/section-2-2.md
```
will become
> # Section 1
>
> section-1.md content ...
>
> # Section 2
>
> # Section 2.1
>
> section-2/section-2-1.md content
>
> # Section 2.2

Huh. This is a pretty big one. And it will just drop mkdocs_combine/ in
here right next to mkdocs_pandoc/ - that absolutely doesn't look right.
