add-roles-permissions: v3.2.0 — remove attribution layer entirely

Removes the attribution layer from add-roles-permissions entirely. Kyra: "Let's remove everything tied to attribution here. We can tackle it separately."

Removed

• permissions.md §2 Attribution section removed. Sections 3-8 renumbered to 2-7.
• "Who wrote what" piece from "The pieces you'll be working with" (was item 4 of six; list is now five items).
• Phase 2 outcomes attribution bullet ("Every save in the team's brain gets stamped with who wrote it").
• Phase 7 setup-complete message no longer mentions attribution.
• Phase 4 readback prose: "wrong attribution easy to spot" → "wrong ownership easy to spot."
• README primitive Add authenticate-apps: reusable login scaffolding for sandbox apps #4 (Attribution) removed; subsequent primitives renumbered. "Six primitives" → "five primitives" throughout.
• README "identity and attribution still useful" softened to "identity resolution alone is still useful."
• marketing.md attribution bullet removed.
• install-config.json description no longer mentions attribution.
