For example the variable in setup is waitMs=1500 next in some condition i wont save the waitMs=2000

I'm sorry, I don't quite understand. Are you asking for a way for an Arduino Sketch to modify the .cfg file? If so, I recommend you call please look at Arduino code to write a file. For example https://www.arduino.cc/en/Tutorial/ReadWrite.

Another way to have multiple values for a single variable is to have multiple variables. For example, you could create a .cfg file that contained "shortWaitMs=1500" and "longerWaitMs=2000". Your Sketch could use shortWaitMs or longerWaitMs, depending on some condition.

I am also wondering if it would be possible to update the configuration file on the SD card. I can't seem to change a value on the SD card. Writing to the SD card adds a new line to the configuration file and in the long run I would have variables saved multiple times in new lines.

Any ideas how to change a value instead of adding new lines?

I'd suggest reading the variables in, deleting the original file, creating a new file with the same name, then writing out the variables.
