Create core ledger cursor table

Context

The poller needs durable progress tracking before it can safely resume after restarts or failures.

Scope

• Define the ledger cursor schema
• Add migration for cursor storage
• Track source/domain and last processed ledger
• Add useful timestamps and indexes

Out of Scope

• Unrelated package work
• Docker, CI, or deployment work unless named in scope
• Broad refactors outside the touched package

• Relevant tests are added or updated
• Documentation is updated when needed
